Resolved Question: How to report a co-worker without suffering retaliation?

I work in a nursing home as a Certified Nursing Assistant. The past few weeks I have been working on one of the long-term wings with a new CNA that was just hired a few weeks ago. During my shifts, I have noticed many things about this worker that bother me. I have found many residents without their bed alarms on or their TABS alarms attached to them. I’ve brought this to her attention, but she is very dismissive about it and simply says “I forgot.” It bothers me because eventually someone is going to fall and we won’t find them soon enough because their alarm won’t be on! Whenever I respectfully correct something that she is doing incorrectly, she gets snippy and defensive. Also, right in the middle of doing a hoyer transfer, she’ll leave the room if a call light goes on. She drops everything to answer the call lights, so our cares last forever and we all end up clocking out WAY too late.I know that morally and ethically I need to report her behavior….but how? The nurse won’t do anything, so I know I’ll have to go to my supervisor. But if this co-worker finds out I turned her in….it’s going to be even worse to work with her. How should I go about this?
